Budi Gunadi: Gov't Yet to Discuss Hike in BPJS Health Insurance Premiums

TEMPO.COJakarta - Minister of Health, Budi Gunadi Sadikin, stated that while the government has not yet discussed an increase in BPJS Kesehatan or national health insurance premiums, it plans to inject Rp20 trillion into the system to maintain its financial sustainability.

Budi conveyed this during a working meeting with the House of Representatives (DPR) Commission IX. "Regarding the planned premium increase, the government has not yet discussed the increase," said Budi Gunadi in the meeting room at the DPR building in Senayan, Jakarta, on Thursday, November 13, 2025.

He explained that the injection of tens of trillions of Rupiah is crucial to stabilizing the financial condition of BPJS Kesehatan. Without these funds, the financial stability and quality of the organization's health services would be affected.

"We are aware that BPJS may not be sustainable. That's why Rp20 trillion is provided to maintain the financial sustainability of BPJS," Budi noted.

He further detailed the funding allocation: Rp10 trillion has already been allocated to the Health Ministry and is ready for disbursement. The remaining Rp10 trillion is currently held by the Finance Ministry.

"That's why I told Pak Ghufron (President Director of BPJS Kesehatan), 'let's speed up the process,' so that it can be transferred to BPJS by January without any delay," Budi emphasized.

Historical Deficits and Sustainability Concerns

Previously, Budi Gunadi stated that BPJS Kesehatan is not sustainable in the long run. Historically, he explained, the financial condition of BPJS Kesehatan has only been positive when premiums are raised, while periods without premium increases have consistently led to deficits.

Based on data from the Health Ministry presented during the working meeting, the National Health Insurance (JKN) system has continuously experienced deficits except for 2019 and during the peak of the COVID-19 pandemic.

Budi provided a detailed breakdown: "Indeed, BPJS has never been sustainable. It is positive when the premiums are increased. So, the premium increase is always late. Negative, negative, negative; when [the premiums] increased, it was positive in 2016. Then, there was COVID-19 in 2021, 2022, which was positive. It turned negative in 2023, 2024, and September 2025."

According to the data, BPJS Kesehatan only recorded positive revenue in 2016, 2019, 2020, 2021, and 2022. All remaining years recorded a deficit. The last premium increases occurred in 2015–2016 and 2020.

Financial Burdens

Since 2023, JKN has consistently faced a deficit. In 2023, BPJS Kesehatan premium revenue amounted to Rp151.7 trillion, while JKN payment expenditure reached Rp158.9 trillion. The deficit pattern continued in 2024, with premium revenue at Rp165.3 trillion and JKN payment expenditure at Rp175.1 trillion. Furthermore, as of September 2025, premium revenue was Rp129.9 trillion, contrasting with a total expenditure burden of Rp139.4 trillion.
